INSTRUCTIONS

• Select the type of permit you wish to purchase and print out an application card for that permit type. Prior to purchase, please visit the Parking Permit Eligibility web page to see what parking permit(s), if any, you are eligible to purchase.
• You can either type in your information directly on the application card or print it out first and fill it in by hand. Please remember to complete the entire application and sign it.
• Deliver the completed application card to our office in person or via mail for processing. Our office is located at 160 Bevis Hall, 1080 Carmack Road, Columbus, OH 43210.
• We do not accept applications faxed to our office due to the poor quality which results in difficulty reading the application.

(PLEASE NOTE: All application cards are in PDF format and require Adobe Acrobat 5.0 or greater. You may download a copy of Adobe Acrobat by clicking on the link below. If you are using Adobe Acrobat Reader, information entered on the application card WILL NOT be saved if you save the file on to your computer. Make sure you print the application card before closing or saving the file. Do not attempt to e-mail your completed application card. Information you entered will not be sent with the file.)

APPLICATION CARDS

• Faculty and A&P Staff - This application card has options for all faculty and A&P staff parking permits.
• Staff - This application card has options for all staff parking permits.
• Visitor - This application card allows visitors to the University to obtain a temporary parking permit.
• Bicycle - This application is for anyone bringing a bicycle to campus.  Bicycle registration is not mandatory, but is strongly recommended. There is no charge for bicycle registration.
• Disability - New This Year - The disability parking option has been added to all application cards. Customers who need an OSU disability parking permit may fill out one of the application cards listed above. Faculty and A&P Staff who need an OSU disability permit may fill out a Faculty and A&P Staff application card. Staff who need an OSU disability permit may fill out a Staff application card.
